Diamond State Health Plan (DSHP)

The DSHP is Delaware’s Medicaid Managed Care Program.
Eligibility is determined by your gross monthly income, the age and number
of family members in your home. There are special programs for pregnant
women, infants and young children, disabled children, disabled workers,
Medicare recipients and people who receive Social Security Supplemental
Income benefits. Delaware Medicaid allows deductions for child care
expenses and each working adult in the home. Additional deductions are given
to self-employed persons, pregnant teens and step-parent families. There are
no monthly premiums or co-pays.

Many
working adults and families qualify for one of the many Medicaid programs,

Delaware Healthy Children Program (DHCP)

DHCP is a low cost health insurance for working families. Delaware children
up to the age of 19 may be eligible based on their parent's gross monthly
income and number of family members in the home. Like Medicaid, deductions
are given for child care expenses, child support received and the number of
working adults in the home. Income limits for DHCP are higher than those of
Medicaid.
Basic DHCP benefits include, but not limited to:

Immunizations
Inpatient and Outpatient Hospitalization
Lab Services
Pharmacy Coverage
Behavioral Health
Eye Exam and Glasses
There is a monthly premium between $10-$25 per family based on income.

Community Heathcare
Access Program (CHAP) / Voluntary Initiative Program (VIP)

CHAP provides access to primary
care doctors, medical specialists, and help with access to other health
resources including prescription programs, laboratory and
radiology services. Medical services are provided in the community through
Community-based Health Care Centers and private doctors who participate in the
Medical Society of Delaware’s Voluntary Initiative Program (VIP). VIP is
a network of private physicians statewide who accept CHAP patients into their
practices and serve as their health home or provide medical subspecialty
services. CHAP recipients receive discounted medical services based upon their
income. In brief, the CHAP program gets patients the medical attention they need
at an affordable cost.
To be eligible for CHAP, an individual must be a resident
of Delaware, uninsured, ineligible for state
medical assistance programs, and meet financial eligibility guidelines. To find

CHAP is
administered by the Delaware Health Care Commission with funding support from
the Delaware
Tobacco Settlement Funds. VIP is administered by
the Delaware Foundation for
Medical Services, a supporting foundation of the Medical Society of Delaware,
with funding support from the Delaware Foundation for Medical Services, the
Medical Society of Delaware and the DHCC.

Delaware's
new Cancer Treatment Program provides treatment for qualified individuals
who can't afford it.
Dealing with cancer is difficult enough. Having it threaten your life
savings and livelihood can cause an even greater personal crisis. So
Delaware created this program to provide cancer treatment for people who
don't have health insurance and who are not eligible for Medicaid.
The Delaware Cancer Treatment Program may help you pay for cancer treatment
for a period of one year if you:
- Are a Delaware resident,
- Have been diagnosed with cancer on or after July 1, 2004,
- Have no comprehensive health insurance, and
- Have a household income that meets qualification guidelines, which are
650% of the Federal Poverty Level (for example, the income for a family of
four must be less than $122,525).

Delaware Prescription Assistance Program (DPAP)

DPAP provides
prescription coverage to the elderly and or disabled adults who do not have
prescription coverage based on gross monthly income and/or have prescription
costs greater than 40% of their monthly income.
This program covers medically necessary prescription drugs. The program does not
pay for any of the drugs or diabetic supplies for Medicare recipients. Medicare
currently covers these supplies for both insulin and non-insulin dependent
patients. Clients must make a co-payment of $5 or 25% of the cost of the
prescription, whichever is greater. The co-pay is collected by the dispensing
pharmacy.
Eligibility:
Clients must reside in the State of Delaware. They must be at least 65 years
old or qualify for Social Security Disability benefits. The income
eligibility limit is set at 200% of the Federal Poverty Level (FPL) which is
currently $1,432 in gross monthly income for one individual. Couples are counted
as two individuals. Individuals with income over 200% of FPL can
qualify if they have prescription costs exceeding 40% of their income. Those
age 65 and over who are eligible for the Nemours Foundation prescription
benefit are not eligible for DPAP. DPAP does not replicate coverage for
Nemours clients.

come into our office to apply. Applicants will need to provide proof of
income with their signed and dated DPAP application. |

Screening for Life

Screening for Life provides
payment for cancer screening tests to qualified Delaware adults. The program
is a cooperative effort of the Delaware Division of Public Health and the
U.S. Centers for Disease Control and Prevention (CDC).
Cancers can be treated more effectively if they are found early, and
Screening for Life can help you get the tests you need at the recommended
times.
What Services Does the Program Provide?
Eligible individuals can receive:
- Office visits
- Mammograms and clinical breast exams for breast cancer
- Pap tests
- Colorectal cancer screening tests recommended by your doctor
- Health education
- Help with coordinating associated care
To be
eligible for these services, you must meet age, income and insurance

and ask
for Screening for Life . If you
are eligible, you will be enrolled and mailed information on how to schedule
your appointment with a participating health care provider. Screening for
Life will help you get the recommended exams for your age and health history
– free or at a cost you can afford.
Who Should Have The Exams?
Pap Tests
: All women age 18 and older, especially if
they are sexually active, should have an annual Pap test and pelvic exam.
Breast Exams: All women should have their breasts examined
by a doctor or other health professional once a year.
Mammograms : Women age 40 and older should have regular
mammograms, as recommended by their doctors or health professionals.
Colorectal Tests: Men and women age 50 and older should have
regular colorectal cancer screening tests, as recommended by their doctors
or health professionals. These tests may include colonoscopy, sigmoidoscopy,
or home-test kits (called FOBT, for fecal occult blood test). |

Eligibility for these services are based on
factors such as who lives and eats together as well as relationships in the
home and income. All children under the age of 22 years old who live with
